The PHP will develop plans to meet your individual mental health needs. Covered
services may include intake/assessment, crisis intervention, support groups, counseling
and/or case monitoring and interpretive services as needed. Services are also
available for clients who are mentally ill and homeless as well as clients who are
mentally ill and involved in the criminal justice system.
Your first visit with a mental health service provider usually involves an intake
evaluation. This evaluation is used to determine which mental health services are
medically necessary and to develop your Treatment Plan which will identify goals and time
frame for service.

- Hospital Liaison and Discharge Planning

- Children's Hospitalization Alternative Program (CHAP) Services
- Children's Long-term Inpatient Program (CLIP)
- Case Management
- In-home Services
- Mentally Ill/Chemically Abusing (MICA) Services
- Housing/Residential Services
- Pre-vocational/Vocational Services
- Interpretive Services
In addition, all individuals residing within the NSMHA service area may receive the
following services:
- Crisis Response
- Involuntary Evaluation
- Involuntary State Hospitalization
- Involuntary Community Hospitalization
